We look forward to helping you.  At your first appointment, you’ll meet the team, Dr. Colleen Allen, discuss any concerns you may have and get an initial exam. Our goal is to make you feel safe, taken care of, and confident in your smile! You’re in good hands with Autumn Dental.


1. Fill out these forms before your appointment:

If this is your first appointment with Autumn Dental, you’ll want to have your medical history paperwork completed. You can access those forms by clicking the buttons above.

This saves you time waiting in our office and allows us to get to know you before your appointment. Having a complete medical history prior to your appointment also gives time to gather information from your insurance company or previous offices and provides time to fill in any gaps in your dental history. Being able to answer these questions prior to your visit makes your appointment as streamlined as possible.

2. COVID Self-Check

– Are you experiencing any of the following symptoms?
  • Fever or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea
– Have you been in contact with anyone that tested positive within the last 24 hours?
If you’ve answered yes to either of these questions, call us and we will find a new appointment day and time. 


3. Arrive 10-15 minutes prior to your appointment with a mask in hand

When you arrive

  • When you arrive for your appointment, you may come into the office. We schedule appointments for minimal waiting and overlap with other patients in the waiting room. If you would prefer, you can also wait in your car and we can call you when it is time for your appointment. If you’d like to wait in your vehicle, please call us when you arrive.
  • If you have been vaccinated, please bring your vaccination card with you so that we may keep that information on file.
  • If you have not been vaccinated, we will take your temperature when you arrive. If you do have a fever at the time of your appointment, we will ask you to reschedule.


We have a team that is dedicated to communicating with your insurance company for the best coverage. In addition to cash, credit cards and checks, we also accept Care Credit.


  • If you are able to fill out your new patient forms before your appointment our team will make the phone calls to your insurance company ahead of time so that we have estimates for your benefits and coverage ready for you at your appointment.
  • If you are unable to fill out your forms ahead of time and do so at your appointment, we can still make the phone calls during your appointment. Many times however, it may take 2-3 days to gather the estimate for coverage.

during your appointment

All patients, vaccinated and unvaccinated alike, will be required to wear a mask in the office unless actively receiving treatment. Initial appointments last approximately 30 minutes to one hour.
  • You will begin by completing the necessary paperwork if you have not previously done so. Find these forms here. 
  • Once you’re called in for your appointment, our hygienist will sit down to get to know you.
  • Next, we will take any needed x-rays. Then, our hygienist will evaluate your perio and oral health and will perform a cleaning if appropriate.
  • If you haven’t been seen by a dentist in some time, it is possible that we may spend the majority of your appointment gathering information about your current oral state, and then may need to reschedule your actual cleaning. This is to ensure we have a complete picture before we begin and allow us to make the most informed decisions about your mouth.
  • Dr. Allen will then join your appointment to perform an oral cancer screening, your full oral exam, and to discuss oral history and concerns. If needed, Dr. Allen will then create and discuss your treatment plan with you.


Our patients’ and team’s health and safety is our number one concern here at Autumn Dental, so we have made changes in our office to ensure that you can still safely come into our office. Along with increased cleaning and sterilization, we have implemented daily staff screenings, and wear additional PPE (Personal Protective Equipment). 
You can be sure that when you come into our office that your health is our top-most concern.
